This plan offers protections against accidents anywhere in the world, on or off the job, on business, on vacation or at home. Air travel is included only while you and your family members are flying as passengers. Some exclusions apply. Employees can choose coverage for themselves or add coverage of dependent family members. |

| Business Travel Accident Coverage is an employer provided benefit that pays you or your beneficiary(ies) if you die or are injured as the result of an accident while you are traveling on business. Maximum benefit payable is three times your total compensation. |

| Basic AD&D Pays benefits to you or your beneficiary(ies) if you die or are injured as a result of an accident. Basic AD&D coverage of one times your eligible earnings is provided by your employer. |

| Optional Basic AD&D Additional AD&D insurance for yourself at one times your eligible earnings to a maximum of $1,500,000 combined with Basic AD&D (per insured person). The monthly premium amount is combined with Optional Basic Life: $0.13 per $1,000 for life insurance + $0.015 per $1,000 for AD&D. |
